Seasonal PMDD/PMS and hormonal binge eating, wine drinking and anxiety – symptoms ramp up from October

Someone in the community asked for help for the seasonal aspect of her PMDD symptoms i.e. more severe symptoms in winter  …

My PMDD symptoms massively increase around the beginning of October right through to March. Would love some advice.

The binge eating and wine drinking is becoming habitual

She specifically mentions binge eating and wine drinking but I assume her other PMDD symptoms – such as anxiety, tension, anger, irritability, depression, sadness, hopelessness, insomnia, overwhelm, low energy, breast tenderness, headaches, pain, bloating, and/ or weight gain –  are more severe at this time too.

As you can see, her symptoms are much more severe from October through March (winter in the Northern Hemisphere). If you live in the Southern Hemisphere you could expect more severe symptoms from May through September (or thereabouts).

There is a seasonal aspect to PMDD (premenstrual dysphoric disorder) and PMS (premenstrual syndrome and other hormonal imbalances.  It’s not well recognized despite the fact that it’s documented in the research and seen clinically.

My feedback for her is to look into and address low serotonin, low GABA and/or low endorphins with the respective amino acids (adjusting up and down based on the season and symptoms), using recognized SAD (seasonal affective disorder) approaches and addressing pyroluria if needed (the nutrients are cofactors for neurotransmitter production.) I share more about each of these approaches and the research below.

The research: patients with PMDD have substantial seasonal patterns in mood and premenstrual symptoms

Premenstrual dysphoric disorder (PMDD) “accounts for the most severe form of PMS with the greatest impairment of women’s functioning and perceived quality of life, often prompting them to seek treatment.”

This older paper from 1997, Seasonality of symptoms in women with late luteal phase dysphoric disorder

  • Out of 100 patients treated in a subspecialty clinic in a university teaching hospital, “a significantly higher rate of seasonal affective disorder (38% versus 8%) as determined by Seasonal Pattern Assessment Questionnaire criteria.”
  • Also, 25% of the patients with seasonal variations in their premenstrual symptoms, consider them marked or a severe problem

The authors conclude that:

These results suggest that patients with LLPDD [or PMDD] have substantial seasonal patterns in mood and premenstrual symptoms. These seasonal patterns have implications for the clinical assessment and treatment of LLPDD [or PMDD]. For example, light therapy may be beneficial for women with seasonal worsening of LLPDD [or PMDD].

As you look at the research keep in mind the fact that late luteal dysphoric disorder (LLDD),“is now known as premenstrual dysphoric disorder (PMDD)”, as mentioned in the above paper and other older studies.

Seasonal variations in serotonin and GABA

Seasonal variations in serotonin have long been documented with much research on the winter blues. This paper, The chronobiology and neurobiology of winter seasonal affective disorder describes winter seasonal affective disorder (SAD) as “a mood disorder characterized by the predictable onset of depression in the fall/winter months, with spontaneous remissions in the spring/summer period.” They also state that “The typical patient with SAD is a premenopausal woman who experiences carbohydrate craving, hypersomnia, and prominent fatigue during winter depressive episodes.”

There is less awareness about GABA seasonality but GABA levels may also be lower in the winter months. In this animal study, Effect of the pineal gland on 5-hydroxytryptamine and γ-aminobutyric acid secretion in the hippocampus of male rats during the summer and winter, it is reported that: “GABA secretion in the hippocampus of rats had a seasonal rhythm consisting of increased secretion in summer and decreased secretion in the winter.”  I share more about this and the seasonality of GABA here.

It makes sense that supporting these lower levels in winter is going to help with the more severe PMDD symptoms that correlate with each neurotransmitter imbalance.

One solution: address low levels of neurotransmitters with amino acids

As you can see the PMDD symptoms mentioned above could fall into the categories of low GABA, low serotonin and/or low endorphins (and possibly low dopamine/catecholamines and low blood sugar too: binge eating, wine drinking, anxiety, tension, anger, irritability, depression, sadness, hopelessness, insomnia, overwhelm, low energy, breast tenderness, headaches, pain, bloating, and/ or weight gain.

It’s for this reason I always have clients with PMDD (and other hormonal issues) do the Amino Acids Mood Questionnaire and trials of the respective amino acids.

Using a food mood log and tracking what time of day you binge eat and drink wine (and  all the other symptoms) is a clue as to which amino acids may help most. I would expect tryptophan, GABA and DPA would be at the top of the list, and possibly tyrosine and glutamine too.

Because of the seasonality aspect, a higher dose is likely to be needed in the winter time so if you’re just starting your amino acid trails keep this in mind. If you are already using amino acids with success in summer, then bumping up the amounts during winter is going to help further. And then be sure to reduce amounts once winter is over.

Hopefully you’ve also been working on diet, gut health, liver health, adrenal function, toxin removal and other factors to reduce or eliminate PMDD/PMS so the amino acids are not needed long term.

How targeted individual amino acids may help – some examples

As you can see from one study, tryptophan can help with premenstrual dysphoria/sadness, mood swings, tension, and irritability.

Low endorphins and low dopamine may also be a factor. I share more on this blog: DLPA (DL-Phenylalanine) eases PMDD/PMS symptoms in women who experience declining endorphin levels in the second half of their cycles

Here are some specific amino acid cases around binge eating/cravings and excessive wine drinking:

  • Would using 5-HTP or tryptophan help when you crave sugar (as a sort of antidepressant) late afternoon/evening?
  • Tryptophan had the added benefit of turning me completely off alcohol when I took it to improve mood and sleep during perimenopause
  • GABA for ending sugar cravings (and anxiety and insomnia)

None of the above are specific to seasonal changes in symptoms but illustrate the use of amino acids.

Supportive solutions: a SAD lamp when serotonin is low and addressing pyroluria

The late luteal phase dysphoric disorder paper above doesn’t mention amino acids (which is unfortunate but not surprising)  but it does mention light therapy. I do recommend the use of a SAD lamp (full spectrum light) when there are low serotonin symptoms that get more severe in the winter.

With PMDD/PMS and other hormonal imbalances, we also always consider pyroluria and the use of higher amounts of zinc, vitamin B6 and evening primrose oil. The “stress” of winter and increased sugar consumption can deplete zinc and vitamin B6 for everyone but more so if you have pyroluria. These nutrients are also key for hormonal imbalances.

Additional resources when you are new to using amino acids as supplements

We use the symptoms questionnaire to figure out if low GABA or low serotonin or low endorphins or low dopamine or low blood sugar may be an issue with your seasonal PMDD/PMS.

“Potion” of tyrosine, Endorphigen, GABA and tryptophan has been nothing less than a miracle for my depression and anxiety – how long can I remain on these?

Amy shares how much her particular “potion” or combination of amino acids has helped her depression and anxiety and wants to know how long she can remain on them:

I currently take 500 mg of l-tyrosine in the morning, 1 Endorphigen 2xday, 1 GABA calm 2x day and 1000 mg of tryptophan before bed with some natural calm & true calm. This “potion” has been nothing less than a miracle. I feel pretty good.

The past few years have been rough, managing depression and anxiety with frequent relapse, and I finally feel ok.

I don’t want to wean off yet and am curious as to how long I can remain on these. Is it dangerous to keep on it too long? Is there a maximum time? I follow your blog and reached out to you last year. Your information has been extremely helpful. Thank you.

I’m glad to hear Amy is doing so well on this miracle combination and finally feels ok. Before stopping the amino acids, the goal is to address diet, gut health, nutritional imbalances, toxins and all the underlying factors that lead to low catecholamines, low endorphins, low GABA and low serotonin.

It’s fine to stop taking them and see how you do and add some or all of them back if you find you still need that neurotransmitter support. This is one of a few approaches I use with clients. But here are times when it’s best to wait and continue using them (like when on the pill and when affected by seasonal depression). I share my insights on this and other ways to cut back below.

How long can you remain on amino acids and when do you consider stopping them?

There is no set time frame for staying on the amino acids. As I mentioned above you can stop taking the individual amino acids and see how you do and then add some or all of them back if you find you still need that neurotransmitter support. This is one of a few approaches I use with clients.

Other approaches include: completely stopping one amino acid at a time and monitoring symptoms or reducing the amount one amino acid at a time, also monitoring symptoms. I prefer the latter especially with someone like Amy who has been “managing depression and anxiety with frequent relapse.”

I can also hear that she has some hesitation about stopping her “potion” and going with your gut is a good thing too. She also has a few confounding factors that make me feel staying on them longer may be in her best interest. One factor is her long-term use of birth control pills.

Long term use of birth control pills and the impact on mood and nutritional status

Amy also shares about her use of the birth control pill and how she suspects it’s a factor in her depression and anxiety:

I should also mention I just went off birth control pills. I have taken them since age 16 only stopping for 3 pregnancies and breastfeeding. I am 46. I’ve been on for painful periods. I made the decision to stop for fear of clots, cardiac issues and most importantly my mental well being. I truly believe they contributed to my depression. I believe I’m in perimenopause and have higher than normal anxiety. I’m extremely worried about my periods if I stop the pill and think my amino acid supplements help these worries and I cope better. That’s why I’m reluctant to stop the supplements but still worried about long term use.

Because Amy has been using the birth control pill for around 25 years I’d have her continue with the amino acids for now and address the effects on her gut health and nutritional status first.

Women taking the pill and other hormonal contraception are more likely to be depressed. A Danish study published in JAMA Psychiatry of more than 1 million women aged 15 to 34, with no prior history of depression, were included in the study and followed for 13 years. The researchers found that those women who used the pill were 23% more likely to be depressed and  use antidepressants.

You can read more about the effects of the birth control pill on this blog and my book review of “Beyond the Pill” by Dr. Jolene Brighten. As you’ll read it causes vitamin, mineral, and antioxidant depletion (such as folate, B12, vitamin B6 and zinc).

She did actually come back and share that she tried and it didn’t work out going off the birth control pill. She was unable to manage the pain. She needs to address all these root causes first.

There is also no reason for her to be worried about long term use of the amino acids when they are clearly addressing her neurotransmitter imbalances.

The seasonal impact on mood and anxiety

I don’t have clients make changes to their amino acids in the Fall or Winter when there can be seasonal impacts on mood and anxiety. She had shared this too:

I’m more prone to depression this time of year. I think with these 2 factors [seasonal mood changes and the pill] I’ll stay on the regime until the spring.

This is a wise decision for Amy.

I would also hold off on making changes if someone is prone to airborne allergies. This is because there can be a higher need for neurotransmitter support when there are allergies.

Amy has already made many diet and lifestyle changes so she is heading in the right direction:

I have given my diet an overhaul. Not perfect but significantly less refined carbs and sugar. Plenty of fresh produce, healthy fats and protein ( especially animal). I also have committed to daily “body movement”: yoga, Pilates, max trainer, light weights or bike.

All of these changes are starting to support her overall nutritional status and resilience.

She is using 500 mg of l-tyrosine in the morning. This supports low levels of dopamine/catchecolamines and improves the curl-up-in-bed kind of depression. You can read more about tyrosine here.

She is also using Endorphigen twice a day. This is a Lidtke product called Endorphigen and provides 500 mg  DPA/d-phenylalanine  per capsule. It raises endorphin levels, offering a mood boost, typically helping with weepiness and also emotional eating. You can read more about DPA here.

Keep in mind this is what Amy found works for her unique needs and would have done trials of the amino acids one by one to figure out which ones to use and how much of each to use. There is no one size fits all.

It’s not unusual for folks using the amino acids to describe their results as miraculous!

Multiple sclerosis: low endorphin research and the amino acid DPA (d-phenylalanine) for pain, depression, comfort and trauma support

The research on the role of low endorphins in multiple sclerosis (MS) is exciting because it creates more awareness about a powerful way to offer pain and mood support if you have been diagnosed with this condition. Typically, I ignore the diagnosis when assessing for low levels of neurotransmitters (via a symptoms questionnaire) and have clients do a trial of the amino acid DPA (d-phenylalanine) if they have physical pain symptoms, experience emotional pain symptoms with excessive weepiness/crying and seek comfort via treats/rewarding foods or the numbing effects of alcohol. However, we now know low endorphins play a role in MS (via the endogenous opioid system). By addressing low levels with DPA, you can find some relief of the above pain/depression symptoms and a need for comfort and numbing. DPA may also offer some trauma support if past trauma is a contributing factor (more on all of this below).

Low endorphins play a role in multiple sclerosis: the research

This 2021 paper, Multiple Sclerosis and the Endogenous Opioid System describes MS and the fact that current therapies have limited efficacy: “Multiple sclerosis (MS) is an autoimmune disease characterized by chronic inflammation, neuronal degeneration and demyelinating lesions within the central nervous system. The mechanisms that underlie the pathogenesis and progression of MS are not fully known and current therapies have limited efficacy.”

What is exciting is the identification of the role of the endogenous opioid system and specific opioid peptides in MS:

Preclinical investigations using the murine experimental autoimmune encephalomyelitis (EAE) model of MS, as well as clinical observations in patients with MS, provide converging lines of evidence implicating the endogenous opioid system in the pathogenesis of this disease.

In recent years, it has become increasingly clear that endogenous opioid peptides, binding μ- (MOR), κ- (KOR) and δ-opioid receptors (DOR), function as immunomodulatory molecules within both the immune and nervous systems.

The endogenous opioid system is also well known to play a role in the development of chronic pain and negative affect [i.e. depression], both of which are common comorbidities in MS. As such, dysregulation of the opioid system may be a mechanism that contributes to the pathogenesis of MS and associated symptoms.

Endogenous means internal i.e natural compounds produced by the body and involved in pain relief and mood improvement. This article, Opioid Peptides, describes peptides as compounds that “produce the same effects as the chemicals known as classic alkaloid opiates, which include morphine and heroin.”

It also mentions three major categories of opioid receptors – mu, delta, and kappa – referred to as MOR, DOR and KOR above.

D-phenylalanine for human “endorphin deficiency diseases”

Unfortunately neither of these papers mentions the amino acid DPA (d-phenylalanine) and the fact that it supports endorphin production (by inhibiting the breakdown of endorphins), reducing pain and improving mood – quickly (as in 5-10 minutes).

The use of DPA is not new information as you can read in this paper from 1982 – D-phenylalanine and other enkephalinase inhibitors as pharmacological agents: implications for some important therapeutic application

A number of compounds have been shown to inhibit the degradation of enkephalins. As expected, these compounds produce naloxone reversible analgesia and potentiate the analgesia produced by enkephalins and by acupuncture.

One of these, D-phenylalanine, is also anti-inflammatory.

D-phenylalanine has proven to be beneficial in many human patients with chronic, intractable pain. It is proposed the enkephalinase inhibitors may be effective in a number of human “endorphin deficiency diseases” such as depression, schizophrenia, convulsive disorders and arthritis.

Such compounds may alleviate other conditions associated with decreased endorphin levels such as opiate withdrawal symptoms.

Prevalence of anxiety/depression and alcohol abuse in MS

As I shared in the recent post addressing low GABA symptoms (anxiety, muscle stiffness, swallowing/voice issues and pain) in multiple sclerosis, anxiety and depression is common in this condition. Alcohol abuse is also high. I shared this paper, The incidence and prevalence of psychiatric disorders in multiple sclerosis: A systematic Review, with the following results:

Among population-based studies, the prevalence of anxiety was 21.9% (and up to 35.0% in some papers), 23.7% for depression …and 14.8% for alcohol abuse.

The above Opioid Peptides paper highlights that the endogenous opioid system may be related to excessive alcohol-drinking behavior. In the work I do with amino acids, I see alcohol used as a way to numb out.

All this supports the fact that the amino acid DPA may help ease symptoms of depression and weepiness seen in MS, and self-medicating with alcohol.

The goal is to use these amino acids instead of needing to use benzodiazepines (covered in the above GABA blog), antidepressants and pain medications.

DPA may help trauma in MS, and the freeze response

This paper, Childhood Trauma in Multiple Sclerosis: A Case-Control Study, suggests an association between childhood trauma and early-life stress and MS:

Although childhood trauma was not associated with the degree of current MS-related disability, patients with MS with histories of physical and/or sexual abuse had significantly higher relapse rates than patients without early-life stress.

DPA may also offer some trauma support if past trauma is a contributing factor. I learned about trauma and the low energy freeze state (a survival mechanism) from Dr. Aimie Apigian, MD, MS, MPH. There is the feeling of numbness and being disconnected when in the freeze state and this eventually becomes the default pattern that the nervous system has been wired into.

Individuals with low endorphins are often in the freeze state and are more emotionally sensitive to everything and because of this they experience much more stress. They also experience a feeling of numbness and feel disconnected. The encouraging news is that the amino acid DPA helps ease the low endorphin symptoms while they are addressing their trauma in other ways, like with somatic work and addressing other biological underpinnings of trauma.

DPA is comforting, helps you feel safe and is often described as feeling like someone just hugged you.

Endorphins and the amino acid DPA (d-phenylalanine) and DLPA (dl-phenylalanine)

Low GABA and low serotonin are common in multiple sclerosis too

Low endorphins are just the tip of the iceberg when it comes to the underlying neurotransmitter imbalances in MS. Low GABA and low serotonin are common too.

As mentioned, I recently blogged about the GABA research and applications of GABA when it comes to multiple sclerosis. Here is that link.

When that blog was published I had a number of questions (see the comments in the above link) from folks asking if GABA could help with similar symptoms in Parkinson’s: swallowing and voice problems, pain and hand spasms. I said yes – if GABA is low, the amino acid GABA will help. As important as your diagnosis is, it’s always the questionnaire/symptoms that help you figure out if it’s worth trialing GABA, DPA or one of the other amino acids.

Both GABA and DPA can help pain symptoms via different mechanisms, so it’s a matter of doing a trial of each amino acid, one at a time and monitoring your response.

Tryptophan and/or 5-HTP may help ease some of the low serotonin worry-type of anxiety, fear, panic attacks, obsessing, low mood and MS-specific pain issues and insomnia.

If you do have more than one imbalance (which is not unusual), you need to figure out which imbalance you have and address that with the relevant amino acids, one at a time. I have clients pick the area that is more problematic for them and start there.

I gathered some of this research while preparing for an interview with the wonderful Dr. Terry Wahls, MD and author of “The Wahls Protocol.” We were both pleasantly surprised to see these endorphin/MS and other neurotransmitter connections.

I really look forward to seeing future research on the use of the amino acids DPA, GABA and tryptophan in MS. And I’d love to be involved in some studies if you are associated with a research facility or do research.

Transgenerational transmission of aspartame-induced anxiety and changes in glutamate-GABA signaling: a new study and solutions

Do you drink diet sodas or use other sugar free products that contain aspartame? Or did you when you were younger? New research has found aspartame induces anxiety and fear via changes in glutamate-GABA signaling. What is even more concerning is that these effects are transgenerational, meaning you could be living with the anxiety-provoking effects of aspartame via the diet soda habits from your father and his father/your grandfather. This study found it impacts 2 generations via the paternal line but I suspect ongoing research may yet find impacts via your mother and grandmother too. Also, keep in mind, your current or prior use of aspartame is very likely going to impact your children and grandchildren. It’s an animal study but we don’t want to ignore yet another issue with artificial sweeteners.

The good news is that we can use the amino acid GABA to help counter the high glutamate and low GABA, and ease this physical anxiety this imbalance causes. And we can use GABA and glutamine and other amino acids to break the addiction to aspartame and other artificial sweeteners.

Here is the abstract of the paper – Transgenerational transmission of aspartame-induced anxiety and changes in glutamate-GABA signaling and gene expression in the amygdala

We report the effects of aspartame on anxiety-like behavior, neurotransmitter signaling and gene expression in the amygdala, a brain region associated with the regulation of anxiety and fear responses. C57BL/6 mice consumed drinking water containing 0.015% or 0.03% aspartame, a dose equivalent of 8 to 15% of the FDA recommended maximum human daily intake, or plain drinking water. Robust anxiety-like behavior (evaluated using open field test and elevated zero maze) was observed in male and female mice consuming the aspartame-containing water. Diazepam, an allosteric modulator of the GABA-A receptor, alleviated the anxiety-like behavior. RNA sequencing of the amygdala followed by KEGG biological pathway analysis of differentially expressed genes showed glutamatergic and GABAergic synapse pathways as significantly enriched. Quantitative PCR showed upregulation of mRNA for the glutamate NMDA receptor subunit 2D (Grin2d) and metabotropic receptor 4 (Grm4) and downregulation of the GABA-A receptor associated protein (Gabarap) mRNA. Thus, taken together, our diazepam and gene expression data show that aspartame consumption shifted the excitation-inhibition equilibrium in the amygdala toward excitation. Even more strikingly, the anxiety-like behavior, its response to diazepam, and changes in amygdala gene expression were transmitted to male and female offspring in two generations descending from the aspartame-exposed males. Extrapolation of the findings to humans suggests that aspartame consumption at doses below the FDA recommended maximum daily intake may produce neurobehavioral changes in aspartame-consuming individuals and their descendants. Thus, human population at risk of aspartame’s potential mental health effects may be larger than current expectations, which only include aspartame-consuming individuals.

A few notes to highlight:

  • Aspartame caused anxiety in both male and female mice but the intergenerational effects were along the paternal line i.e.“the anxiety phenotype, its response to diazepam, and changes in amygdala gene expression were transmitted from the aspartame-exposed male founders to their descendants.”
  • These effects were caused by the equivalent to 2–4 small /8 oz/ 240 ml cans of aspartame-sweetened diet soda.
  • The anxiety-inducing effects of aspartame in humans are potentially far more widespread than what is currently known

There is still no consensus on adverse effects

The paper also highlights a very concerning aspect: there is still no consensus on aspartame’s effects on brain monoamine content (serotonin and the catecholamines dopamine, adrenaline, and noradrenaline) or behaviors such as memory, depression and anxiety. Despite much research, consensus is also still lacking on aspartame’s carcinogenic or cancer-causing effects and the impacts on weight-gain and obesity, and the bad effects on the microbiome.

In fact, “the US government policy and publications indicate that aspartame is safe when consumed within FDA recommended maximum daily intake value of 50 mg/kg.” In this study it was much less than the maximum i.e. the equivalence of just “8 to 15% of the FDA recommended maximum human daily intake” caused the above anxiety and intergenerational effects.

This paper is adding to the body of knowledge but if you’re still consuming aspartame it is time to make your own decisions and quit. This is often easier said than done and I address this below.

GABA and other amino acids as a solution for your diet soda addiction

I address the addictive nature of diet soda in this blog: I need help with my Diet Coke addiction – when I stop, my fatigue, brain fog, anxiety/depression increase big time!

I need help with my Diet Coke addiction. I have been using it for 35 years and I am too appalled to tell you how much I drink.

I have tried to quit many times and ended up in utter failure. When I stop, my fatigue, brain fog, anxiety/depression increase big time. I know this drink is literally killing my health and I am desperate for some answers, suggestions….or a miracle.

Kevin shares this in the comments: “I was VERY addicted to diet coke and Pepsi. I found oxycontin and morphine easier to quit.” And Karen shares this: “I can relate to a Coke/Pepsi (diet and/or regular) addiction because I drank 3 every day, Monday-Friday for years. For me, that first sip in the morning was my “hit” in the morning for this teetotaler.”

When it comes to using amino acids to help break the addiction, pretty much everything I write about in relation to sugar addiction or cravings would also apply to quitting Diet Coke or other diet sodas. You need to figure out what emotions are driving your addiction and use the respective amino acid supplement/s (one or more of them) to help break the addiction and improve your anxiety and mood symptoms at the same time.

This could be glutamine if you drink it when your blood sugar is low and crave the sweet taste, GABA if you drink it when stressed, tryptophan or 5-HTP if you drink it later in the day in order to feel happy, DPA if it’s your reward and tyrosine if it gives you a mood and energy boost. It’s all explained in the above blog post.

The amino acids ease alcohol withdrawal symptoms too. I share a study in this blog – An amino acid supplement with DLPA, glutamine and 5-HTP eases alcohol withdrawal symptoms at an inpatient detoxification program – and practical solutions using these amino acids. Read this and simply substitute alcohol with diet soda. And keep in mind it’s always an individualized approach based on each person’s needs, for endorphin and dopamine support (from the DLPA), blood sugar support (from glutamine) and serotonin support (from 5-HTP).

GABA as a solution for the physical anxiety symptoms

In this study the mice responded well to diazepam, a benzodiazepine prescribed for anxiety. However, given the many issues with long-term benzo use, the amino acid GABA, used as a supplement is a safer choice. It also addresses the root cause of low GABA and high glutamate.

The FDA approved aspartame as a nutritive sweetener in 1981 and for use in carbonated beverages in 1983 so I don’t have the intergenerational impacts. I drank Tab in my early 20s and it contained aspartame for a short time. It was very likely part of the perfect storm of factors that contributed to my anxiety. GABA was a life-saver for me.

Low lithium questionnaire and how we use lithium orotate with the amino acids

This is the low lithium questionnaire that I use with new clients in order for us to figure out if a trial of low dose lithium, in the form of lithium orotate, may be helpful. The hallmark of low lithium is a rollercoaster of emotions. Keep in mind that this is just one of 12 questionnaires that I have my clients complete. Many of the following symptoms can have multiple causes, the labs may relate to other deficiencies and the conditions have other root causes. This questionnaire simply provides additional evidence that lithium orotate may help.

We typically do a lithium orotate trial, starting with 5 mg once a day, and going up to 10 mg twice a day. We do this after we have started trialing the respective amino acids for low serotonin, low GABA, low endorphins, low catecholamines and low blood sugar. A big clue that lithium orotate may be helpful (when many of the symptoms below are checked off) is when the amino acids for low serotonin (tryptophan or 5-HTP), low GABA (GABA or theanine), low endorphins (DPA or DLPA), low catecholamines (tyrosine or DLPA) and low blood sugar (glutamine) are not as effective as expected (based on the amino acids mood/neurotransmitter questionnaire).

If you are new to low dose lithium / lithium orotate

As I share in this blog, Upping my tryptophan and lithium orotate have been absolutely profound for me: I’ve been depression free and anxiety free for over a year, I’ve used lithium orotate with many clients and use it when folks have mood swings and anxiety ups and downs. It’s harder for the amino acids to work when there is a moving goal post and lithium orotate evens things out.

You can read Katrin’s wonderful results: “Upping my tryptophan dose and also including and upping the dose of lithium orotate has been absolutely profound for me. I’m off my SSRI/antidepressant (which I was off and on for a number of years). I’ve been depression/anxiety free for over a year. So fantastic.”

The above blog also includes additional information on the differences between low dose lithium / lithium orotate and prescription lithium carbonate. The latter is used at much higher doses and does have side-effects.

One of the many ways lithium works is via the impact on neurotransmitter production. This paper, Potential Mechanisms of Action of Lithium in Bipolar Disorder, states this: “At a neuronal level, lithium reduces excitatory (dopamine and glutamate) but increases inhibitory (GABA) neurotransmission.” It also increases protective proteins such as BDNF (brain-derived neurotrophic factor), helps reduce oxidative stress and is neuroprotective. This paper is referring to lithium carbonate and not lithium orotate but until we have more research on lithium orotate, I feel comfortable extrapolating, given what I’ve seen clinically with lithium orotate.

I’ve also blogged about low dose or microdose lithium here: Microdose lithium formulation is capable of halting signs of advanced Alzheimer’s and improving cognition. In a study published in 2020, “a team of researchers has shown that, when given in a formulation that facilitates passage to the brain, lithium in doses up to 400 times lower than what is currently being prescribed for mood disorders is capable of both halting signs of advanced Alzheimer’s pathology and of recovering lost cognitive abilities.”  In this study, they used lithium citrate in similar doses as the lithium orotate i.e  3.2 mg to 6.4 mg NP03 based on 70kg of body weight (which is around 154.3 lbs).

I had not investigated DPA as I am a joyful, happy person. But recently I have been so depleted that I benefitted much from taking licorice to boost cortisol (who knew cortisol isn’t always to be lowered?)

Your presentation convinced me to try DPA as when I get depleted enough I get weepy (not sadness, just from being physically drained). Wow. I used a 500 mg dose of Lidke Endorphigen in the afternoon, and that has made a startling difference. I have more resilience now, more buffer, in the caregiving work I do, and just the oops’s of life.

I have been an avid note-taker of your summits and the interviews you give on the summits of others, and have recommended your book and blog to precious ones looking for answers to their health challenges.

Because of your diligence, I have been greatly helped by using GABA, 5-HTP (seems to work better for me than tryptophan), and tyrosine (so my thyroid glandular works more effectively).

Thank you ever so much for presenting information with evidence backing in a way that truly meets my curious, investigative, analytical mind. I love to know the whys and hows of things.

Ellen shared this feedback on one of the blogs after she heard me talk about DPA and low endorphins on The Anxiety Summit 5: Gut-Brain Axis. I’m sharing her wonderful results to give you a practical resource if you find yourself depleted and needing resilience as a caregiver. I’m also sharing this in order to illustrate that it’s not always easy to know when to trial a certain amino acid.

Ellen commented that she appreciated understanding the whys and hows of things. She is referring to my explanation of how DPA (d-phenylalanine) works. It’s an amino acid that destroys the enzyme that breaks down endorphins and thereby helps to raise your endorphins. With low endorphins you can feel very weepy, overly emotional, be extra sensitive to emotional pain, be sensitive to physical pain, and indulge in comfort/reward eating. You also often don’t feel joy.

Even though she was well versed in the use of GABA, 5-HTP and tyrosine (and seeing results), she had not investigated DPA. She described herself as a joyful and happy person so she didn’t think she needed DPA.

But her caregiving work (and other health and life challenges) had left her depleted and she started to feel weepy. And she is happy to share that DPA reversed that feeling. I’m thrilled for her and appreciate her for sharing.

The DPA product, how best to use it and how it differs from DLPA

Ellen mentions Lidtke Endorphigen which contains 500 mg of DPA. This product has been a long-time firm favorite with my clients and those in my community.

I used to recommend simply chewing the capsule to get the quickest and best effects and this worked well when it was produced in a gelatin capsule. Now it’s made with a cellulose capsule and chewing doesn’t work at all well, so opening the capsule (or just biting off the top) and tipping the powder into your mouth works best.

I’m often asked about the difference between DPA and DLPA (dl-phenylalanine) so if you have this question you can read more about that here.

Some of the supporting research

Research supports the use of DPA for helping to ease depression and the fact that caregivers are prone to an emotional burden.

  • D-phenylalanine and other enkephalinase inhibitors as pharmacological agents: implications for some important therapeutic application

It is proposed that the enkephalinase [a subgroup of endorphins] inhibitors may be effective in a number of human “endorphin deficiency diseases” such as depression…

  • Burden and quality of life of caregivers for hemodialysis patients

Caregivers of hemodialysis patients may experience a significant burden and an adverse effect on their quality of life. Emotional aspects of caregivers (particularly female spouses) and patients are important predictors of burden.

The study mentions that “Social support and psychological interventions should be considered to improve caregiver life and patient outcomes.”

There is no research that I am aware of that has found DPA to be beneficial for caregivers but until we have that research, let’s include nutritional support based on clinical results we see.

This case, Caregiving Burden, Stress, and Health Effects Among Family Caregivers of Adult Cancer Patients, illustrates what many caregivers experience: “extremely high levels of psychological distress, including anxiety, depression, worry and extreme loneliness.”

Ellen was already using GABA, 5-HTP and tyrosine when she added DPA, so as a caregiver she was already addressing her low GABA physical-type anxiety (with GABA), low serotonin worry-type anxiety (with 5-HTP) and low dopamine low-energy/low mood (with tyrosine). Adding DPA was the cherry-on-the-top for her weepiness and low resilience, and gave her more of a buffer.
