Set up your initial complimentary 15 minute phone consultation

Please use this page to set up an initial complimentary 15 minute consultation so we can determine if we are a good fit and so I can help you select the best package for your needs.

There are 3 steps to this process:

  1. COMPLETE THE INITIAL INTAKE FORM: Complete the initial 3 page intake form – this will help me prepare for our session and help guide our conversation.  I’ll also use this information should you decide to work with me.  You’ll complete this online on your computer.

  2. SIGN THE DISCLOSURE: Once you’ve completed the above online intake form, you’ll automatically be taken to a form so you can read about my services and sign the Limit of Services Disclosure.  You’ll simply sign online with your mouse.

  3. BOOK A DAY/TIME TO TALK: Once you’ve signed this Limit of Services Disclosure, you’ll automatically  be taken to my time scheduling system to pick a day/time for our initial consultation.

 

A few pointers

  • You will receive an email with your copy of the completed intake form and a copy of the Limit of Services Disclosure page.  You’ll receive a separate email with your appointment information.
  • This initial complimentary 15 minute consultation will be done over the phone and I’ll be phoning you.
  • You do need to complete all 3 steps outlined in the above sequence. If you don’t, I won’t have the information to call you.
